Foundayo is the brand name for orforglipron, made by Eli Lilly. It belongs to the same GLP‑1 family as Mounjaro and Wegovy, and works the same way — by mimicking the gut hormone that tells your brain you are full — but it is the first of its kind that works as an ordinary tablet rather than a weekly injection. The UK medicines regulator, the MHRA, authorised it in August 2026, making the UK the first country in Europe to approve it.

How the dose builds up

Everybody starts low and steps up slowly, staying at each dose for at least a month. This is what keeps the digestive side effects manageable, and it is not a step to rush.

You must not become pregnant while taking Foundayo. If you take the contraceptive pill it may stop working reliably — you will need a non‑oral method or a barrier method for 30 days after starting and for 30 days after every dose increase.

If you take simvastatin, your dose must not exceed 20 mg a day while you are on Foundayo.

Foundayo is a prescription‑only medicine. Every order is reviewed by a UK‑registered prescriber before anything is dispensed, and it is prescribed alongside a reduced‑calorie diet and increased physical activity — not instead of them. It is not currently available on the NHS.

Results vary from person to person. These are trial averages, achieved alongside a reduced‑calorie diet and increased physical activity. Foundayo works with those changes — not instead of them. Some people lose more, some lose less, and a small number do not respond.

Pregnancy and contraception: what you must know before you start

Essential safety information

You must not become pregnant while taking Foundayo.

Foundayo may harm an unborn baby. It must not be used if you are pregnant, think you may be pregnant, or are trying to conceive. If you become pregnant while taking it, stop taking Foundayo straight away and contact us and your GP or midwife.

If you could become pregnant, you need to use effective contraception for the whole time you are on this treatment.

If you take the contraceptive pill, it may stop working reliably

Foundayo slows down how quickly your stomach empties, which can reduce how much of your contraceptive pill your body absorbs. This is the single most commonly missed piece of advice about this medicine.

What to do

Either switch to a non‑oral method of contraception (such as the implant, injection, coil or patch), or add a barrier method such as condoms.

Do this for 30 days after you start Foundayo.

And again for 30 days after every dose increase — each step up carries the same risk, so this is not a one‑off precaution.

Planning a pregnancy? Speak to us before you start, or before you stop, so we can plan the timing with you. Foundayo is also not suitable while breastfeeding.

Simvastatin: your dose must not go above 20 mg

Do not take more than 20 mg of simvastatin a day while you are on Foundayo.

Foundayo raises the level of simvastatin in your blood. Higher simvastatin levels increase the risk of muscle problems — muscle pain, tenderness or weakness — which in rare cases can become serious and affect the kidneys.

What to do

• Tell us if you take simvastatin — including the exact dose — before you start Foundayo.

• If your simvastatin dose is 40 mg or 80 mg, it will need to be reduced or changed to a different statin. Do not make that change yourself — speak to us or your GP first.

• Report any unexplained muscle pain, tenderness, weakness or dark urine straight away.

Spacing the two medicines a couple of hours apart does not solve this — the interaction still happens. The 20 mg daily limit is what matters.

Which medicines can I not take with Foundayo?

Tell us about everything you take — prescription medicines, pharmacy medicines, herbal remedies and supplements — before you start. Some combinations need a dose change, and some need to be avoided altogether.

Do not take alongside Foundayo

Another GLP‑1 medicine. Never combine Foundayo with Mounjaro, Wegovy, Ozempic, Saxenda or any other GLP‑1. This includes overlapping when switching — we will tell you how to change over safely.

Strong CYP3A4 inducers — for example rifampicin, carbamazepine, phenytoin and St John’s wort. These make Foundayo much less effective.

Strong CYP3A4 inhibitors that also block OATP1B — such as ciclosporin. These push Foundayo levels up sharply.

Needs a dose adjustment or close monitoring

Simvastatin — maximum 20 mg a day. See the dedicated simvastatin section above.

Strong CYP3A4 inhibitors — for example clarithromycin, ketoconazole, itraconazole and some HIV medicines. If one is unavoidable, your Foundayo dose must not go above 9 mg a day.

Insulin and sulfonylureas (such as gliclazide) — the combination can drop your blood sugar too low. The dose of those medicines often needs reducing, and this must be done by your diabetes team.

The contraceptive pill — may become unreliable. See the pregnancy and contraception section above.

A general point about tablets. Foundayo slows your stomach down, so any medicine you swallow may be absorbed more slowly. If you take something where the exact level matters — warfarin, thyroid tablets, epilepsy medicines — tell us, so it can be monitored.

What is Foundayo and how does it work?

Foundayo is the brand name for orforglipron, made by Eli Lilly. It is a GLP‑1 receptor agonist — the same family as Mounjaro and Wegovy — but it is the first one that works as an ordinary tablet rather than a weekly injection.

GLP‑1 is a hormone your gut releases when you eat. It tells your brain you are full and slows how quickly food leaves your stomach. Foundayo mimics that signal, so you feel satisfied sooner, stay full for longer, and find it easier to eat less without constant hunger.

Unlike some other oral treatments, there are no food or water restrictions and no need to take it first thing in the morning. You can take it whenever suits your routine — as long as it is around the same time each day.

How do I take Foundayo, and how does the dose go up?

Swallow one tablet whole, once a day, with or without food. Everyone starts on the lowest dose and builds up slowly — this is deliberate, and it is what keeps side effects manageable.

The maximum dose is 17.2 mg a day. Not everyone needs to reach it. If you are doing well and tolerating a lower dose, staying there is perfectly reasonable — we will review this with you at each reorder.

Each pack contains 30 tablets — one month’s supply at one tablet a day.

Switching from Mounjaro, Wegovy or another weight loss injection

You start at the beginning — 0.8 mg a day — no matter which medicine you were on before, how long you took it, or what dose you had reached.

There is no dose that “matches” your old one. Foundayo contains orforglipron, which is a completely different molecule from the ones in the injections. Mounjaro (tirzepatide) and Wegovy (semaglutide) are peptides given by injection once a week; orforglipron is a small molecule you swallow every day. Because they are built differently and behave differently in the body, no equivalent dose has been established between them — so there is no way to carry your progress across as a starting dose, and guessing one would not be safe.

What this means in practice

You begin on 0.8 mg and work up through the same steps as someone who has never taken a weight loss medicine before — at least 30 days on each step.
Never take both at once. Stop the injection before you start Foundayo. Taking two GLP‑1 medicines together sharply raises the risk of severe sickness, dehydration and low blood sugar.
Timing matters. The injections stay in your body for several days after a dose. We will normally start you on Foundayo on the day your next injection would have been due — your prescriber will confirm this for you.
Tell us what you were taking — the medicine, the dose, and the date of your last one. Please include this when you order: the prescriber needs all three to get your start date right.

What to expect while you build back up

Appetite suppression will probably feel weaker at first than it did on your previous full dose, and your weight may hold steady for a while rather than falling. That is expected — it is the titration doing its job, not the medicine failing to work. It is also why we do not rush the steps.

What are the side effects?

Most side effects are digestive, are worst when you start or step up a dose, and settle as your body adjusts. Going up slowly is the best way to avoid them.

Common (more than 1 in 20 people)

Feeling sick • constipation • diarrhoea • being sick • indigestion • tummy pain • headache • bloating • tiredness • burping • heartburn • wind • hair thinning

Stop and get medical help urgently if you have

• Severe, persistent tummy pain, especially if it spreads to your back and you are being sick — this could be inflammation of the pancreas.

• A lump in your neck, difficulty swallowing, shortness of breath or a hoarse voice that will not go away.

• Signs of an allergic reaction — swelling of the face, lips or throat, or difficulty breathing.

Around 3 in 100 people had a severe digestive reaction in the trials, compared with 1 in 100 on placebo. Keeping your fluids up, eating smaller meals and avoiding rich or fatty food helps a great deal in the first few weeks. Always read the patient information leaflet in your pack for the full list.

Who should not take Foundayo?

Foundayo is not suitable if you

• Have ever had medullary thyroid cancer, or anyone in your family has.

• Have multiple endocrine neoplasia type 2 (MEN 2), a rare inherited hormone condition.

• Are pregnant, trying to conceive, or breastfeeding.

• Have had a serious allergic reaction to orforglipron or any ingredient in the tablet.

In animal studies, medicines of this type have been linked to thyroid tumours. It is not known whether this applies to people, but as a precaution Foundayo is not prescribed to anyone with that personal or family history.

Tell us as well if you have had pancreatitis, gallstones, diabetic eye disease, kidney problems, or a history of an eating disorder. These do not automatically rule you out, but our prescriber needs to know before deciding whether Foundayo is right for you.

Am I eligible, and what happens after I order?

Foundayo was authorised by the UK medicines regulator (the MHRA) in August 2026 for weight management in adults. It is a prescription‑only medicine, so every order goes through a clinical assessment before anything is dispensed.

You may be eligible if you are an adult with

• A BMI of 30 or above, or

• A BMI between 27 and 30 together with a weight‑related health condition, such as high blood pressure, type 2 diabetes, high cholesterol or sleep apnoea.

Complete the online assessment. Answer honestly — it is what your prescriber relies on.

A UK‑registered prescriber reviews it, and may come back to you with questions.

If approved, your medicine is dispensed and posted discreetly. If not, you are refunded in full and told why.

Foundayo is not currently available on the NHS — NICE has still to complete its review — so it is offered privately. Placing an order is a request for treatment, never a guarantee that it will be approved.
